PROPOSAL FOR RETIREMENT FACILITY:
The myPHR card is a credit card sized card with a cardholders
name and ID number on it. The myPHR card will have an ID number
which can be used to access the health information and insurance
billing information of the cardholder. The card is easily
updated whenever any changes occur.
Health professionals use mobile wireless devices to access the
information from any location. Wireless devices will be
pre-programmed so very little time is required to log into the
system and can even be done enroute to an emergency. (Access
will only get better as technology and wireless devices become
more advanced.) Health professionals can also access a patients
information using any computer with access to the internet at
the myPHR website. An emergency responder logs in a then inputs
the ID number of the patient to retrieve the member's
information.
By entering the member's ID number at the data is displayed on
the mobile internet device screen in any participating fire
truck, ambulance, emergency department, doctor's office,
pharmacy, outpatient surgical facility, or retirement home, etc.
The computer screen display is easily printed to paper.
The benefits to your organization are:
Whenever your residents are transported to the emergency room or
their doctor's office, the myPHR card will contain all of the
health and insurance information needed by the health care
provider. It might be read in a participating ambulance,
emergency department, or doctor's office.
A printout of the card could be produced by your facility for
the resident to hand carry in case there might not be an
authorized health professional where they are going. This will
increase efficiency and shorten the time spent with each patient
thereby making the evaluation quicker.
If the patient is unable to answer the Doctor's questions for
whatever reason then, with the myPHR card, detailed information
is still available. This may be life saving such as in instances
of unconscious patient.
Insurance billing information is provided for up to two carriers
with ID numbers and telephone numbers.
You could offer the myPHR card at your expense to your
prospective residents as an incentive to lease with you. This
would then be a marketing tool to sell your services.
You could assist your residents in obtaining the myPHR card at
their own expense and then provide for them a computer in your
facility so members can update their information and so
printouts can be made to accompany them to their health care
providers.

that you are able to read the health information of your
patients who are participants of this program. If you have
access to the internet in your facility you will also be able to
access and enter participants health information. If you

on an instruction sheet which is provided to ALL card holders
and the myPHR website. This will be free advertising for your
organization.
