Steps to create a Personal Health Record:

Create an account

Log into member's area

After you enter your information and press "submit" you will be assigned a 12 digit membership number.

Print your own wallet card online, print your own records to take to your doctors and make sure your emergency contacts also have your login information

Note: You do not have to enter all your information at once, only your name and address is required initially. You can log back in and add and update your information online at any time.
